UTI & skin/soft tissue

  • Nitrofurantoin (Macrobid)100 mg twice daily × 5 d

Note: Nitrofurantoin is first-line for uncomplicated cystitis in non-pregnant adults. Avoid if CrCl <30 or if pyelonephritis is suspected.
Respiratory + antivirals

Sinus, throat, flu

  • Amoxicillin500 mg TID × 10 d
  • Augmentin875/125 mg BID × 10 d
  • Azithromycin (Z-Pak)500 → 250 mg × 5 d
  • Oseltamivir (Tamiflu)75 mg BID × 5 d
Flu antivirals: Oseltamivir works best when started within 48 hours of symptom onset. Don't wait if you're symptomatic during flu season.
Steroids + adjuncts

Anti-inflammatory bursts

  • Medrol dose pack4 mg taper × 6 d
  • Prednisone20 mg daily × 5 d
  • Benzonatate (Tessalon)200 mg TID for cough
  • Ondansetron (Zofran)4 mg ODT for nausea
Steroid bursts: Short courses (≤7 days) typically don't need a taper. Useful for asthma flares, allergic reactions, severe sinusitis.

When to skip us and go to the ER

Urgent telehealth handles common acute conditions. It's not appropriate for true emergencies:

  • Difficulty breathing, chest pain, severe shortness of breath
  • Stiff neck with fever and severe headache (possible meningitis)
  • Sepsis criteria: high fever + confusion + fast heart rate
  • Severe abdominal pain, blood in vomit or stool
  • Spreading red streaks from a wound, or fever + spreading infection
  • Pyelonephritis: flank pain, high fever, nausea/vomiting with UTI symptoms — go to ER

Do I need to be seen in person for a UTI?+
For uncomplicated UTIs in non-pregnant women, asynchronous telehealth is well-supported by guidelines. Classic symptoms + no red flags = empiric antibiotic. Complicated cases (pyelonephritis, recurrent, pregnant, male) need in-person.
Will you treat strep without a culture?+
We can treat empirically for likely strep based on symptoms (Centor criteria) and timing. For ambiguous cases your provider may ask you to do a rapid strep test at a local lab or urgent care before prescribing.
What about COVID?+
Antiviral (Paxlovid) requires a positive test and specific criteria. We can discuss your situation and order Paxlovid where appropriate, or symptomatic care otherwise.
Antibiotic stewardship?+
We don't hand out antibiotics for viral colds. If symptoms are clearly viral, we'll explain why an antibiotic won't help and recommend symptomatic care.
What if I get worse on the antibiotic?+
Message us. If you develop high fever, vomiting, spreading infection, or worsening after 48-72 hours of antibiotics, you may need to switch or be seen in person. Don't wait it out silently.
