When to skip us and call 911 instead

PDE-5 inhibitors are very safe but a few situations require immediate care:

  • Chest pain or pressure during or after taking a PDE-5 — STOP and call 911. Do not take nitroglycerin within 24-48 hours of a PDE-5.
  • Erection lasting longer than 4 hours (priapism) — go to the ER. Untreated priapism can cause permanent damage.
  • Sudden vision or hearing loss after taking a PDE-5 — stop the med, go to the ER.

Sildenafil or tadalafil — which one?+
Sildenafil works in 30-60 min and lasts 4-6 hours. Tadalafil works in 30-60 min and lasts up to 36 hours. Tadalafil also has a daily low-dose option that gives a steady on-state. Vardenafil is similar to sildenafil. Your provider helps you pick based on your history and lifestyle.
I take blood pressure medication — is this OK?+
Most blood pressure meds are fine with PDE-5s. The dangerous combo is nitrates (nitroglycerin for chest pain). Alpha-blockers (tamsulosin for BPH) need to be timed carefully. Your provider will review your full med list.
What about insurance?+
Generic sildenafil and tadalafil are often cheaper as cash-pay than going through insurance. Many GoodRx-style prices are under $20 for a month. Branded Viagra/Cialis can be expensive without insurance coverage.
Will I get hard every time?+
PDE-5s help the blood vessels respond — they don't create arousal. You still need normal stimulation. If something doesn't work for you, your provider can change the dose, switch the medication, or refer you to a urologist.
Can I take this with alcohol?+
Modest alcohol (1-2 drinks) usually doesn't cause problems. Heavy alcohol blunts arousal and increases the risk of a blood pressure drop. Use common sense.
What if my provider declines?+
If your provider declines, they'll explain why and tell you where to go next — usually a cardiologist clearance, urologist referral, or in-person evaluation. We never push a prescription that isn't safe for your situation.
