Pharmacy Participation Agreement
Summary of binding terms
By signing below, you (the "Pharmacy") agree to participate in the PrescriberNow network operated by PrescriberNow LLC, a Florida limited liability company, on the following terms:
1. Credentialing. You authorize PrescriberNow to verify your state pharmacy license, DEA registration, NCPDP, NABP scorecard, and PIC license. You'll respond to credentialing requests within 5 business days.
2. Compliance. You'll dispense only valid, complete prescriptions; comply with USP 795/797/800 and applicable state pharmacy law; and maintain PHI security under HIPAA.
3. Patient handling. You'll fill PrescriberNow Rx within reasonable turnaround (target 48 business hours) or notify PrescriberNow if you cannot fill, so we can re-route.
4. Fees & payment. Participation requires a $49/month membership subscription, which begins with a 14-day free trial. Your card is collected today but not charged during the trial; the first $49 charge occurs 14 days after signup and automatically every month thereafter until cancelled, cancellable per the cancellation terms below. Cancel any time before the trial ends to avoid being charged. You continue to bill the patient directly (cash) or insurance per your standard process, and PrescriberNow does not take a percentage of Rx revenue.
5. Term & termination. Either party may terminate with 30 days' written notice. PrescriberNow may suspend access immediately for credentialing failure, patient safety incidents, or material breach.
6. Indemnification. Each party indemnifies the other for its own negligence and HIPAA violations.
7. Governing law. Florida law. Venue: Martin County, FL. Binding arbitration per AHLA rules for disputes over $25,000.

Membership payment
Free-trial & recurring billing authorization. By checking the box and clicking submit, you start a 14-day free trial and authorize PrescriberNow LLC to charge your card $0.00 today, then $49.00 on day 14 and $49.00 automatically every month thereafter as a recurring subscription, until you cancel. This is an auto-renewing membership with no fixed end date. To cancel, email team@prescribernow.com any time — cancel before the trial ends and you are never charged; after that, cancellation stops future monthly charges (already-billed months are non-refundable). Card details are securely tokenized by Authorize.Net — your full card number never touches PrescriberNow's servers.
