Your Rights

You're in control of your health record.

Federal law (HIPAA) gives you the right to see your medical record, fix mistakes in it, and find out who we've shared it with. Use the forms below to make a request. You won't need a lawyer, and there's no charge for routine requests.

Get a copy of your records 45 CFR 164.524

You can ask us for a copy of your medical record at any time. We can send you a PDF by secure email, mail a paper copy, or send it to another doctor for you. Tell us what you need and we'll take care of it.

We respond within 30 days

Fix a mistake in your record 45 CFR 164.526

If something in your record is wrong or incomplete, you can ask us to correct it. Tell us which visit, which field is wrong, what it should say, and why. Your provider will review and reply.

See who we've shared your record with 45 CFR 164.528

You can ask for a list of times we shared your record outside of normal treatment, payment, or operations — for example, sharing required by a court order or public-health authority. Pick a date range and we'll prepare the list.
